This is how I did it using Windows Vista and MushClient.

1) Tested my IPv6 capabilities here: http://test-ipv6.com/

Initial report said: Your IPv6 address on the public Internet appears to be 2001:0:4137:9e76:c29:280d:252b:xxxx Your IPv6 service appears to be: Teredo

You appear to be able to browse the IPv4 Internet only. You will not be able to reach IPv6-only sites.

Your IPv6 connection appears to be using Teredo, a type of IPv4/IPv6 gateway; currently it connects only to direct IP's. Your browser will not be able to go to IPv6 sites by name. This means the current configuration is not useful for browsing IPv6 web sites. [more info]

Your DNS server (possibly run by your ISP) appears to have no access to the IPv6 Internet, or is not configured to use it. This may in the future restrict your ability to reach IPv6-only sites. [more info]

2) Downloaded the gw6c client from http://www.go6.net/4105/download.asp (the 32-bit version).

3) Installed the 32-bit gw6c client, ran it and clicked "Enable".

4) Tested my IPv6 capabilities again with http://test-ipv6.com/

Report now said: Your IPv4 address on the public Internet appears to be 218.186.14.xxx

Your IPv6 address on the public Internet appears to be 2001:5c0:1000:x::xxx Your IPv6 service appears to be: freenet6

Congratulations! You appear to have both IPv4 and IPv6 Internet working. If a publisher publishes to IPv6, your browser will connect using IPv6. Note: Your browser appears to prefer IPv4 over IPv6 when given the choice. This may in the future affect the accuracy of sites who guess at your location.

Your DNS server (possibly run by your ISP) appears to have no access to the IPv6 Internet, or is not configured to use it. This may in the future restrict your ability to reach IPv6-only sites. [more info]

5) Opened Command Prompt using admin permissions (right-click Command Prompt, click Run as Administrator) and typed the following into it:

netsh interface portproxy add v4tov6 listenport=4242 connectaddress=discworld6.obsidian.org.uk connectport=4242

6) Opened MushClient, opened the Discworld file, opened World Properties and changed the MUD TCP/IP address to localhost, port 4242. Clicked ok.

7) Clicked connect and I was connected by IPv6 instead of IPv4.

You last logged in from cm12.sigma102.xxx.xxx.sg (::ffff:218.212.102.xx) and are currently logged in from 2001:5c0:1000:a::xxx (2001:5c0:1000:a::xxx).
