It is possible to carve some items. This seems to be purely cosmetic.

To carve or sharpen an item you need to be holding a knife.

The following are items that can be customized by carving or something similar.

Elephant tusk

The elephant tusks sold in an ivory stall in the Djelibeybi bazaar can be carved by someone with enough skills. Carving it uses crafts.carpentry.whittling and crafts.arts.design.

Every time you look at the tusk you'll either see for the last line "This tusk would be of great use to an individual with such skills." which means you can't do any design or a random design each time you look at it, with more designs available if you have more skill. The skill checked might be crafts.arts.design.

Elephant tusks such as this one are highly prized by wealthy dilettantes across the Disc for the intricate designs that can be produced in the ivory by a suitably skilled craftsman.  You could try to carve <design> on it.

You can "carve <design> on <object>" while holding a knife and having the other hand free. This uses crafts.points. The more gp it takes the harder it is to carve.

This will change the last line to the chosen design with skill qualifiers.

If you don't have enough skill it damages the tusk: "Properly carved, it would be much sought after by discerning connoisseurs, but someone has already tried to do so and botched the job so badly that it isn't practical to salvage it."

If you try to carve a tusk on the ground or maybe without enough gp or if you stop while carving it changes it to "This tusk, has the beginnings of a carving of <design> on it. A suitably skilled craftsman might be able to complete the design." With enough skills you can finish it.

If you succeed it will be the result as below, with two qualifiers that are probably skills dependent:

The designs you can see are limited by your crafts.arts.design bonus, allowing you to see all those that have the same gp cost at the same skill.
